FCPS Emergency Care Form

FCPS Field Trip Driver's License and Vehicle Insurance
FCPS Parental Authorization and Acknowledgement of Risk
SC Crew Liability
VHSL Physical Form (turn into SCSS Activities Office)
SC Crew Swim Certificate (not required for returning rowers)

PLUS, in addition to the 8 (yes 8!) forms above, there is also US Rowing waiver form that must be completed online and forwarded to SC Crew (see below).

NOTE:  VHSL Physical form: Complete before Winter Conditioning begins!  Submit completed, signed form to SCSS Student Activities Office.  Must be based on a physical conducted after May 1, 2010. Submit completed / SIGNED form to SCSS  Student Activities office.

ADDITIONAL FORM:

U.S. Rowing waiver form:  Complete by January 1, 2011, online.
